The appropriate applicant must be willing to start as an Independent Contractor with a limited number of hours to start.*
Job Purpose: To provide physical therapy services to clients within their homes. Duties & Responsibilities: •Schedules client sessions •Attends client sessions at client homes as scheduled •Provides physical therapy to manage pain, improve movement, and/or prevent loss of mobility •Completes daily SOAP notes in EMR and tracks goals accordingly •Conducts initial evaluations (IE) per company guidelines •Completes re-evaluations every 90 days or as directed •Updates relevant documentation, diagnoses, communications, etc. in EMR as needed •Establishes and executes physical therapy program accordingly to assessment, physician’s orders (i.e. Rx & Dx), and client goals/objectives •Prepares for and participates in rehabilitation team meetings whenever necessary •Proactively plans and executes discharge per client progress •Maintains at least 85% overall client attendance by developing proactive corrective plans to address client attendance concerns •Communicates with FTU staff regarding client care concerns •Follows professional standards, safety standards, and state and federal regulatory requirements
•Must be a Licensed Physical Therapist in the State of Michigan •Must be willing to work within client homes and as an Independent Contractor (1099) •Homecare physical therapy experience is a plus •Capable of successfully performing core competencies •Demonstrates leadership skills and problem-solving abilities •Must display the ability to exercise independent judgment based on experience to implement effective treatment combinations •Highly developed interpersonal, communication, and time management skills •Excellent organizational skills •Must be able to read, write, and speak English •Must be able to carry out directives and multi-task in a fast-paced environment •Must be able to work independently as well as with a team •Must be extremely self-motivated and keep up with the demands of a growing company •Must have strong computer skills, including knowledge of Microsoft Office
